Pharmacological properties

Indapamid ((rs) - 4-chloro-3-sulfamol-n-(2-methyl-2.3 dihydro-1n-indole-1-silt) benzamide) — diuretic, antihypertensive medicament of group of not thiazide sulfonamides. indapamid causes decrease in a tone of unstriated muscles of arteries due to change of transmembrane current of calcium ions, decrease in sensitivity of a vascular wall to noradrenaline and angiotensin that leads to decrease opss. indapamid stimulates prostaglandin synthesis e2, having vasodilating and hypotensive effect. shows moderate diuretic action which is connected with blocking of a reabsorption of ions of sodium, chlorine, water and, to a lesser extent — potassium ions and magnesium. indapamid does not break metabolism of lipids and has no negative impact on carbohydrate metabolism, does not influence the level of the general hs and the tenge, reduces the level of phosphates in blood serum. the therapeutic effect develops in 7–8 days, the maximum effect — in 4 weeks

After intake, the maximum concentration in blood serum is reached in 2-2.5 h and makes 230–260 ng/ml. About 71-79% of an indapamid contact proteins of blood plasma, mainly with α ₁ - a glycoprotein. It is widely distributed in body tissues. Elimination half-life — 14 h. It is metabolized in a liver. It is removed with urine (7% — in not changed look for 48 h, 70% — in the form of metabolites) and with bile (about 23%).

Indication

Use

Adult — on 2.5 mg of 1 times day, is better in the morning. in case of insufficient hypotensive effect the dose is raised to 5 mg a day (in one step in the morning). increase in a dose of medicament enhances diuretic effect.

Contraindication

Hypersensitivity to an indapamid and other components of medicament and also to streptocides, acute disorders of cerebral circulation, a hyper aldosteronism, the profound abnormal liver functions and kidneys, a hypopotassemia.

Side effects

Headache, increased fatigue, spasms, diarrhea, skin rash, the speeded-up urination, in isolated cases — orthostatic hypotension.

Special instructions

With extra care the patient with an abnormal liver function and kidneys. in the course of medicament treatment the periodic control of level of electrolytes in blood serum in connection with a possibility of development of a hypopotassemia and disturbance of water and electrolytic balance is necessary. periodic control of level of uric acid in blood serum is necessary for patients with a hyperuricemia and gout which apply indapamid.

. Due to decrease in the ABP, medicament can affect ability to control of vehicles and mechanisms. It is not necessary to appoint Indapamid during pregnancy and feeding a breast.

potentiation of effect of other antihypertensive medicaments at simultaneous use with indapamidy Is possible

Interaction. do not appoint along with lithium medicaments as at the same time risk of development of toxic effects of the last increases (owing to decrease in renal clearance of lithium and increase in its concentration in blood plasma). indapamid can reduce efficiency of Norepinephrinum. it is not necessary to appoint indapamid with kaliysberegayushchy (Spironolactonum, amiloride, Triamterenum) diuretics. simultaneous use of kaliysberegayushchy diuretics does not exclude a possibility of development of a hypopotassemia.

Overdose

nausea, vomiting, weakness, dysfunctions of a digestive tract, disturbance of water and electrolytic balance, in hard cases — arterial hypotension and respiratory depression Are possible
