Captopril of the tab. of 25 mg No. 20 Solution Pharm

Captopril - means, affects renin-angiotenzinovuyu a system. APF (APF) inhibitors, monocomponent. Indications

• arterial hypertension;
• heart failure. Captopril is intended for treatment of chronic heart failure with decrease in systolic function of ventricles and also in a combination with diuretics and, if necessary, from a foxglove and β-blockers;
a myocardial infarction for short-term (4 weeks) treatment is possible
• prescribing of captopril within 24 hours after the postponed myocardial infarction to patients with a stable state; for long prevention of symptomatic heart failure medicament is shown to patients with clinically stable state with asymptomatic dysfunction of a left ventricle (fraction of emission ≤ 40%);

• diabetic nephropathy at patients with diabetes of the I type, is shown makroproteinuriyeyu.

Structure

• active ingredient: captopril;
• 1 tablet contains captopril * 25 mg;
• cellulose microcrystalline lactose monohydrate, potato starch; magnesium stearate.

Contraindication

• hypersensitivity to captopril or to medicament excipients and also to other APF inhibitors;
• a Quincke's edema (including in the anamnesis, after use of APF inhibitors, hereditary / idiopathic);
• subaortic stenosis or mitral stenosis, existence of other obstacles to outflow of blood from a left ventricle of heart;
• a hypertrophic cardiomyopathy with low warm emission;
• primary hyper aldosteronism;
• hyperpotassemia;
• heavy renal failures bilateral narrowing of renal arteries or an arteriostenosis of the only kidney a state after renal transplantation;
• congenital (idiopathic) Quincke's disease;
• porphyria;
• intolerance of a galactose, deficiency of lactase, syndrome of glyukozo-galaktozny malabsorption;
• pregnancy; also contraindicated the women planning pregnancy;
• feeding period a breast
• simultaneous use of captopril with the medicaments containing aliskiren, to patients, sick diabetes or to patients with a renal failure (glomerular filtration rate ˂ 60 ml/min. / 1.73 sq.m).

Route of administration

Dosage should be adjusted depending on the nature of a disease of the patient.

Captopril is accepted inside to, in time or after meal. It is necessary to take the medicament regularly, at the same time every day. If it is missed reception of a tablet, it should be accepted as soon as possible; however if there were several hours before reception of the following dose, then the following dose is recommended to be accepted according to the schedule and not to perceive the passed dose. It is not necessary to accept 2 doses of captopril at the same time.

In need of prescribing of captopril in a dose of 6.25 mg should use medicament in the corresponding dosage or in other dosage form.

Arterial hypertension.

Recommended initial dose makes 25-50 mg a day in 2 receptions daily. In 2-4 weeks treatment it is possible to carry out titration of a dose depending on the reached arterial blood pressure to 100-150 mg a day to 2 receptions. Captopril can be applied separately or with other antihypertensive drugs, especially with thiazide diuretics. The mode of dosing of 1 times a day can be applied when such accompanying antihypertensive medicament as thiazide diuretic is added.

Heart failure.

Initial dose - 6.25-12.5 mg 2 or 3 times a day. Titration to a maintenance dose (75-150 mg a day) should be carried out on the basis of reaction of the patient to treatment (data of objective inspection and tolerance of drug). The dose should be increased gradually, with intervals not less than 2 weeks for the purpose of assessment of reaction of the patient to treatment. The maximum daily dose makes 150 mg in 2 receptions.

Myocardial infarction.

Short-term treatment. Use of medicament in the first 24 hours after a myocardial infarction should be carried out according to the following scheme: an initial dose - 6.25 mg, through 2:00 - 12.5 mg and through 12:00 - 25 more mg of captopril. From the next day within 4 weeks, captopril should be accepted in the dose 100 mg a day divided into 2 receptions. At the end of 4 weeks treatment it is necessary to make repeated assessment of a condition of the patient for decision-making on treatment at a stage after the postponed myocardial infarction.

Long-term treatment. If use of captopril is not begun within the first 24 hours of a stage of an acute myocardial infarction, it is recommended to begin treatment during the period between the third and sixteenth days after a heart attack with the moment when necessary conditions of treatment (stable hemodynamics and treatments of any residual ischemia) are provided. Treatment should be begun in hospital under stringent control (in particular, arterial blood pressure) until achievement of a dose of 75 mg a day.

Diabetic nephropathy at the patients sick with diabetes of the I type.

Captopril should apply in a dose 75-100 mg a day in 2 receptions, if necessary - to combine with other antihypertensive drugs.

Efficiency and safety of use of Captopril to children are studied by

insufficiently. Children should begin captopril use under careful medical control. The initial dose of captopril makes 0.3 mg/kg of body weight. For special groups of patients (to children with a renal failure and in case of immaturity of an urinary system) the initial dose has to make 0.15 mg/kg of body weight. Usually captopril is appointed to children by 3 times a day, but the interval between introduction needs to be selected individually, depending on reaction of the patient to administration of drug.
